Every player MUST have an MHSAA Physical dated after 4/15/2022 along with the Athlete/Parent Responsibilty Form & the Henry Ford Consent for Treatment Form to participate in tryouts, practices and/or competitions. NO EXCEPTIONS!!!   

Forms are to be turned into the HS Athletic Office to receive a practice/tryout ticket.  Without a ticket, they are NOT able to participate.   

ALL athletes MUST have a physical dated after 4/15/23 to participate in sports this year. Their physical from last year will not work.
